The use of robotics has been established for manual dexterity, feeding, and locomotion and has been adapted for motor assessment of brain-injured patients. Our aim was to develop a low-cost robotic system for evaluating motor function of patients with poststroke hemiplegia and severe mobility problems. We created a closed-loop controlled robotic system that subjects were trained to use for testing of selected tasks related to automatic movement sequencing, and transferability. Eighteen patients with poststroke hemiplegia and severe mobility problems were recruited. These patients were tested on the robotic system that had 1) a 2-dimensional (2D) flat platform with push buttons and a joy stick on the end to indicate the direction of movement, 2) a stabilizing arm to stabilize the platform, 3) a touch-sensitive screen to display and track the correct response, 4) a computerized closed loop control system, and 5) a battery-driven microcomputer for computing and controlling the robotic system. The robotic system was used to assess upper limb automatic movement sequencing by measuring the time it took subjects to move a 4-pound push button with the touch screen to a target. Lower limb automatic movement sequencing was assessed by measuring subjects’ ability to perform a “cognitive” Stroop task. The effects of the severity of poststroke hemiplegia, age, and sex on the performances were investigated. Robotic examination of automatic movement sequencing was completed by 15 of the 18 participants. The mean time (SD) required to reach the target on the push button was 84 seconds (9.2 seconds). The mean time (SD) for the “cognitive” Stroop task was 116 seconds (28 seconds).
